Abaloparatide
Also called Tymlos, Eladynos, BA058, BIM-44058
Abaloparatide is a lab-made peptide modeled on parathyroid hormone-related protein (PTHrP). Like teriparatide, it is a daily injection that builds new bone, and it is FDA-approved for osteoporosis in women and men at high fracture risk.
Overview
Abaloparatide is a lab-made peptide modeled on parathyroid hormone-related protein (PTHrP). Like teriparatide, it is a daily injection that builds new bone, and it is FDA-approved for osteoporosis in women and men at high fracture risk.
How it works
Established: abaloparatide activates the PTH1 receptor and its cAMP signaling pathway. Daily pulsed dosing stimulates osteoblasts to lay down new bone on both inner (trabecular) and outer (cortical) bone surfaces (FDA label). Proposed: it may favor a receptor conformation that gives a shorter signal than teriparatide, which has been suggested to explain less calcium rise, but this is not established in the label.
What research shows
- 18-month RCT in postmenopausal women (824 abaloparatide vs 821 placebo): new vertebral fractures 0.6% vs 4.2%, an 86% relative reduction (FDA label, Study 003).
- 12-month RCT in men (149 vs 79): lumbar spine BMD +8.5% vs +1.2%, total hip +2.1% vs <0.1% (FDA label, Study 019).
- Rats given abaloparatide developed osteosarcoma in a dose- and time-dependent way; whether this applies to humans is unknown (FDA label).
Doses studied in people
| Study or label | Dose used | Source |
|---|---|---|
| FDA label (Tymlos), postmenopausal women and men with osteoporosis | 80 mcg subcutaneously once daily | 1 |
Label dose is 80 mcg once daily; total lifetime use beyond 2 years is not recommended because longer use has not been studied.
Read this first: these are the amounts given to participants in published studies or written on official prescribing labels, reported for reference. They are not recommendations. Trial doses were chosen for specific patients under medical supervision, and a dose that is safe in one person can be harmful in another.

Safety
- Orthostatic hypotension, usually within 4 hours of injection; first doses should be given where the patient can sit or lie down (FDA label).
- Hypercalcemia and hypercalciuria/kidney stones; not recommended with hypercalcemic disorders (FDA label).
- Osteosarcoma seen in rats; avoid in people at higher risk (open growth plates, Paget's disease, skeletal cancer, prior skeletal radiation) (FDA label).
- Common reactions in women vs placebo: hypercalciuria 11% vs 9%, dizziness 10% vs 6%, nausea 8% vs 3%, palpitations 5% vs 0.4% (FDA label).
- Hypersensitivity, including anaphylaxis and angioedema (FDA label).

Storage and stability
Before first use, refrigerate at 2-8°C; after first use, may be kept at 20-25°C for up to 30 days; do not freeze (FDA label).

History
Abaloparatide was invented at Ipsen (BIM-44058) and developed by Radius Health as BA058. The FDA approved it as Tymlos in April 2017 for postmenopausal osteoporosis; the EU approved it (Eladynos) in December 2022. The FDA removed its osteosarcoma boxed warning around December 2021 based on long-term PTH-class data, and later expanded the label to men with osteoporosis.
